Non-Biased Smoking Survey, No Agenda

As much as I hate myself for starting a smoking thread, I am interested in getting some actual data that isn’t biased by either side of the debate. The polling engine here on CC doesn’t support more than one multiple choice question which makes it darn near impossible to collect all the data you need for meaningful statistical significance. If you would be so kind as to to take the short, six question survey, it will be greatly appreciated.
Some notes: You can only vote once. If there are more than two votes from the same IP address, they will be discarded. This will allow spouses to vote, but excludes children and pets from getting in on the action.
No personally identifiable information is required to participate. The survey will close at noon Eastern Daylight Savings Time on Friday, August 1, 2014.

In order to reduce response bias, there will be minimal readouts of the survey data until the project window closes.
If you are curious about what’s being asked before you click on the link, here are the questions:
- I consider myself a Holland America 1. cruiser. (Yes/No)*
- I have an upcoming Holland America cruise scheduled. (Yes/No)*
- Seven choices to describe your balcony smoking attitude. The seven choices should cover all the bases. Just pick the one that best describes you.*
- Are you a smoker, non-smoker, or former smoker?*
- Your location live, state & country only.
- Age group
*-Questions 1 through 4 are required for your response to be considered. Questions 5 and 6 are optional, but will help when we compile the data.
Results, with pretty graphs, will be published here at the conclusion of the survey. If you don’t vote, you voice will not be heard.
The survey engine will not argue with you, nor will it call you names. If you want to do that, there is plenty of space below. Knock your socks off.
